Before you attend each football session you MUST carry out a self-screen checklist. For all under the age of 18, a parent carer or guardian should complete this on behalf of each child. This is a requirement by the Football Association for all players taking part in grassroots football.
Answer the questions below to the best of your knowledge.
If you answer "NO" to this question, you (or your child) may attend the football activity.
If you answer "YES" to this question, you (or your child) are not able to partcipate. Do not travel to the football session. Follow applicable Government Guidance.
1. Does the person attending the session have any of the following symptoms:
A high temperature (above 37.8°C)
A new continuous cough
Shortness of breath
Loss of or change in sense of taste or smell
Feeling generally unwell
Being in close contact or living with a suspected or confirmed case of Covid-19
Part of a group that has been asked to self-isolate (e.g. a school class, another sports team)
It is important to remember some people can pass on the virus before they develop symptoms, or never have symptoms despite being infectious. Despite everyone’s best efforts, these participants cannot be screened out of our football sessions.
